Athletics Registration & Sports Physicals

  Athletics Registration  
RankOne is our online platform for athletics registration, forms & sports fee collection.
All sports physicals are required to be uploaded to this platform and you will sign all Student Eligibility Forms through RankOne. *NO PAPER PHYSICALS WILL BE ACCEPTED.

Please read this step-by-step guide on the DIAA website on creating a parent account as well as navigation instructions. 

Once logged in, you will be required to enter your Student’s First Name, Last Name and DOB (Date of Birth) to claim your student(s). You can claim multiple students within one account. You do not need to create multiple accounts if you have more than 1 student-athlete. Once you set up your account, you will not have to set it up again.

The required DIAA & ASPIRA eligibility forms will now be electronically signed through this portal. Your child’s sports physical will need to be uploaded to the “Sports Physical Upload Form” within RankOne.

There will be 5 forms you need to complete inside of RankOne:
  1. DIAA Eligibility Form – must be complete before proceeding with other forms
  2. Sports Physical Upload Form- Upload Page 3 (History Form), Page 4 (Physical Examination Form) & Page 5 (School Athlete Medical Card) of the Physical form – jpeg or pdf. This must be done as 1 single document containing all of these pages.
  3. Pre-Participation Consent Form - This consolidated form includes the Sudden Cardiac Arrest Awareness, Heat Policy Awareness, the Concussion Awareness, & Consent to Treat.
  4. Emergency Contact Card
  5. DIAA/ASPIRA Athletics Acknowledgement Forms
Double-check that each form is submitted and shows as “Complete.” (green)

It is recommended for parents and guardians to download the RankOne App. The app allows you to access all forms and information for your student from your phone. Once your student-athlete is placed on a team roster in Rank One, you will then have access to that team’s information and communication from the school and coaching staff.

  Sports Physicals  
All athletes must have a valid DIAA sports physical dated after April 1, 2026 in order to participate. 
All sports physicals are required to be uploaded to RankOne. 
*NO PAPER PHYSICALS WILL BE ACCEPTED.

Una vez que inicie sesión, deberá ingresar el nombre, apellido y fecha de nacimiento de su(s) estudiante(s) para registrarlo(s). Puede registrar varios estudiantes en una sola cuenta. No es necesario crear varias cuentas si tiene más de un estudiante deportista. Una vez que configure su cuenta, no tendrá que volver a hacerlo.


Los formularios de elegibilidad DIAA y ASPIRA requeridos ahora se firmarán electrónicamente a través de este portal. El examen físico deportivo de su hijo/a deberá subirse en el formulario "Sports Physical Upload Form" dentro de RankOne.

Dentro de RankOne, deberá completar 5 formularios:

  1. Formulario de Elegibilidad de DIAA–Debe completarse antes de continuar con otros formularios.

  2. Formulario de subir los datos del examen físico deportivo - Sube la página 3 (Historial clínico), la página 4 (Examen físico) y la página 5 (Tarjeta médica del deportista escolar) del formulario de examen físico en formato JPEG o PDF. Debes enviar un solo documento con todas estas páginas.

  3. Formulario de consentimiento previo a la participación - Este formulario consolidado incluye la información sobre el paro cardíaco, la política del calor, las conmociones cerebrales y el consentimiento para recibir tratamiento.

Verifique dos veces que cada formulario se envíe y aparezca como “Completado.” (verde)

 

Se recomienda a los padres y tutores descargar la aplicación RankOne. Esta aplicación les permite acceder a todos los formularios e información de su hijo/a desde su teléfono. Una vez que su hijo/a sea incluido/a en la lista de un equipo en RankOne, tendrán acceso a la información y las comunicaciones de ese equipo por parte de la escuela y el cuerpo técnico.

 

  Exámenes Médicos Deportivos  

Todos los atletas deben tener un examen físico deportivo DIAA válido con fecha después del 1 de abril de 2026 para poder participar.

Todos los exámenes físicos deportivos deben subirse a RankOne.

*NO SE ACEPTARÁN EXÁMENES MÉDICOS EN PAPEL.

Winter Season Ticketing

WINTER TICKETING FOR HOME GIRLS & BOYS BASKETBALL GAMES

ELECTRONIC tickets are required for all girls & boys basketball games. There will be NO CASH ticket options available & tickets must be purchased prior to the games. We have limited capacity in the gym and when that capacity is reached, no more tickets will be sold and no more entrants will be allowed in.
  1. Adult & Children Admission: All adults must purchase a $3 GoFan ticket to attend each girls & boys basketball games. Children 12 & under are FREE but you must still reserve a ticket for each child.
  2. ASPIRA Staff & Student Admission: ASPIRA Staff & Students are FREE. However, students will be required to reserve a ticket and must be accompanied by an adult to enter the games.
